Osteopathy van der Aar respects the privacy of its clients and ensures that the personal information you provide is treated confidentially. In this privacy statement, you can read how I handle your personal data. I explain what personal data I collect, for what purposes I collect it and where your data is stored. Your personal data is processed in accordance with the General Data Protection Regulation (GDPR).

Osteopathy van der Aar provides osteopathic care for adults, children and babies in the Amstelveen area and surrounding regions. You can visit Osteopathy van der Aar for osteopathic assessments and treatments at the Keizer Karelpark Health Centre.

Osteopathy van der Aar uses patient data solely for the purpose for which the data was collected. I do not share patient data with third parties, unless this is necessary for the purpose of storage. Osteopathy van der Aar does not retain patient data for longer than is necessary based on the purpose for which the data was collected. I take all possible measures to keep patient data secure from unauthorised access. In addition, I request consent from patients for the storage of personal data if no treatment contract has been concluded. In this privacy statement, I inform patients about their rights regarding their personal data. Furthermore, I inform you about the purpose of the processing of your personal data, and I also inform you if I need to carry out specific actions involving your personal data.

Osteopathy van der Aar processes your personal data because you use my services and/or because you provide them to us yourself. Below is an overview of the personal data I process:

Purpose of data

Osteopathy van der Aar processes your personal data for the purpose of being able to call or email you if this is necessary to provide our services.

1 | Treatment file

Osteopathy falls within the legal framework of primary healthcare. This entails a number of obligations. I am legally obliged to create a proper treatment file and to include your personal data, special categories of personal data and treatment details within it. I process your personal data: full name, gender, date of birth, address, postcode, town/city, telephone numbers, email address, National Insurance number, GP, practitioners and your identity document number. I process your special categories of data: reason for seeking help, pattern of symptoms, medical history, treatments, medication, family situation, lifestyle, hobbies, nature and course of the treatments.

2 | Invoicing

Regulatory bodies and insurers have set requirements that an invoice must meet; for this purpose, I process your full name, date of birth, address, postcode and town/city. To send the invoice to you, I also process your email address.

3 | Correspondence with third parties

At the end of the treatment cycle, I will inform your GP and referring practitioners about the treatment I have provided. This is a report containing our findings, the treatment, the progress of the treatment and the outcome. To ensure accurate correspondence, I process your full name, gender, date of birth, address, postcode, town/city, and the contact details of your GP and/or referring practitioners.

I do not disclose any data to third parties, other than where required by law.

Occasionally, patients ask us to send a treatment report to other practitioners (other than Osteopathy van der Aar and their GP) or to a personal injury insurance company. I will only disclose treatment details upon your explicit and written request as our patient.

4 | Contacting us

When you contact Osteopathy van der Aar via email, the details you provide, such as your name, company name and email address, are stored on the mail server.

When you contact me by telephone, your telephone number is stored in my telephone (switchboard).

5 | Our website

I do not use cookies on my website. As soon as you wish to make an appointment with me and click on the ‘make an appointment’ button, you will be redirected to my calendar managed by Crossuite. The personal data you enter here will be processed with care.

My website includes social media buttons. The administrators of these services use these to collect your personal data. You are personally responsible for clicking these buttons and thereby disclosing your personal data to the administrators of these services. This takes place outside my sphere of influence, and I am therefore not liable for it.

Osteopathy van der Aar uses Google Analytics to track how users use the website. Google uses this information to track how my website is used and to provide me with reports about the website.

Google may provide this information to third parties if Google is legally obliged to do so, or insofar as these third parties process the information on Google’s behalf. This is beyond my control, and I am therefore not liable for it.

Processors

Osteopathy van der Aar processes your personal data because you use my services and/or because you have provided this data to me yourself.

Crossuite

The (special) personal data you provide, the osteopath’s findings and the treatment details are carefully processed by me in Crossuite’s automated system OSTEOnline. Documents and emails you send to me are stored in OSTEOnline, and all documents I create, including appointment confirmations and invoices, are also stored there by me.

Retention periods

I do not retain personal data that I process for longer than is necessary for the purpose for which it was collected or as required by Dutch law. Personal data and special categories of personal data will be deleted if you, as a patient, have not visited for more than 15 years.
